Premium
0.69% from credit card outstanding balance.
Outstanding balance shall refer to the total balance shown in the cardholder's monthly bill, which includes any outstanding fees, penalties (including late payment interest), retail/cash transactions, unbilled outstanding balances and all other debit balances prior to the date of death, disability or diagnosis of critical illness. Claims made on any outstanding balance is subject to the Maximum Sum Assured Amount per credit card type.
Death Benefit
If the Insured dies due to illness or accident within the insurance period, PT Asuransi Allianz Life will pay 100% of the credit card outstanding balance less any indebtedness due, subject to the maximum Sum Assured as set out in the Table of Maximum Sum Assured. Upon payment of the Death Benefit, the policy shall end.
Total Temporary Disability (TTD) Benefit
If the Insured is disabled due to an illness or accident that lasts for more than 30 consecutive days, PT Asuransi Allianz Life will pay 10% of the credit card outstanding balance or minimum Rp50,000 as a Total Temporary Disability Benefit, subject to the maximum Sum Assured as set out in the Table of Maximum Sum Assured. This benefit is accelerated to the Death Benefit and Total Permanent Disability Benefit and will be paid on the 31st day of the disability period. The maximum total temporary disability period is 6 months.
Total Permanent Disability (TPD) Benefit
If the Insured is disabled due to illness or accident continuously for 6 months and determined by a physician that the disability will be permanent, PT Asuransi Allianz Life will pay 100% of the credit card outstanding balance, less any Indebtedness due, subject to the maximum Sum Assured as set out in the Table of Maximum Sum Assured. This benefit is accelerated to the Death Benefit and Total Permanent Disability Benefit and will be paid on the 31st day of the disability period upon payment of the Total Permanent Disability benefit, the policy shall end.
Criticial Illness (CI) Benefit
If diagnosed with any 1 of 10 following critical illnesses:
- First heart attack
- Cancer
- Stroke
- Kidney failure
- Other coronary serious disseases
- Benign brain tumour
- Multiple sclerosis
- Cardiomyopathy
- Coronary artery bypass
- Coma
and has survived 30 days since diagnosis, Allianz will pay 100% Sum Assured of credit card outstanding balance, subject to the maximum Sum Assured as set out in the Table of Maximum Sum Assured. This benefit is in addition to Death Benefit cover. Upon payment of the Critical Illness benefit, the Critical Illness Benefit coverage will end.
In cases where a Critical Illness benefit is paid to the Insured or the Insured is undergoing a survival period since diagnosed with Critical Illness, but death/TPD were to eventuate, the sum assured for death/TPD would be based on the sum assured for the Critical Illness.
No Claims Bonus Illustration
A no claims bonus will be paid to customers every 2 years if a Temporary Disability, Permanent Disability, Critical Illness or Death Benefit claim is not made every two years after the first premium being paid. The amount of bonus that will be paid is 15% of the total premium paid every 2-years period.
For example: he customer paid his/her first premium of Rp150,000 on January 2015. Total premium being paid from January 2015 to December 2016 is Rp3,600,000. If no claims being made for the Temporary Disability, Permanent Disability, Critical Illness or Death Benefit risks during the January 2015 up to December 2016 period, then PT Asuransi Allianz Life Indonesia will pay a no claim bonus of Rp540,000 (which is 15% x Rp3,600,000). However, if the customer filed for a claim during the January 2015 up to December 2016 period, then the customer will not get a no claim bonus from PT Asuransi Allianz Life Indonesia. If the customer paid his/her premium from January 2017 to December 2018 and no claims being made during that period, then PT Asuransi Allianz Life Indonesia will pay again a no claim bonus of Rp 540,000 (which is 15% x Rp 3,600,000).

A 45 years-old customer got an outstanding of Rp1,000,000 on his credit card statement printed on January 2015. The benefits that the customers can get is as follow:

The insured / policy holder must submit the following documents:
- Melengkapi formulir klaim Allianz yang telah diisi lengkap dan ditandatangani oleh Tertanggung dan bagian resume medis diisi lengkap dan telah ditandatangani oleh Dokter yang merawat dengan cap dari Rumah Sakit.
- Accompanied by the relevant form(s) below:
Death Benefit:
- Death claim form (supplied by the Insurer);
- A copy of the credit card statements for 3 (three) months;
- A copy of the Identity Card of the Insured;
- Copy of Family Card of Insured;
- Certificate of treating physicians regarding the diagnosis of the cause of the insured dies;
- Original Death Certificate / certified copy by local district;
- A copy of the HSBC Credit Card;
- Original Letter of the Minutes from the Police in the case of unnatural death or because of a traffic accident;
- Certificate from the local Indonesian Representative Office in the case died abroad.
Total Permanent Disability Benefit (TPD):
- Claim form (provided by the Insurer);
- A copy of the Identity Card of the Insured;
- A copy of the HSBC credit card statements for 3 (three) months;
- Copy of Family Card of the Insured;
- A copy of the HSBC Credit Card;
- Original Letter of the Minutes of the Police / copy legalized in the event of an accident;
- A statement that expressed by the treating physician that the disability will occur for the rest of his life;
- X-rays of the limb defects;
- Allianz fully entitled to ask for the Insured to do medical examination to designated Physician Consultant periodically during the Insured is still suffering from Total Permanent Disability.
Total Temporary Disability Benefit (TTD):
- Claim form (provided by the Insurer);
- A copy of the HSBC credit card statements for 3 (three) months;
- A copy of the Identity Card of the Insured;
- Copy of Family Card of the Insured;
- A copy of the HSBC Credit Card;
- A statement from treating doctor when the Insured begin suffer disability begin when the disability began to heal.
Critical Illness (CI):
- Critical illness claim form (provided by the Insurer) is filled by the Beneficiary or the Insured;
- A copy of the HSBC credit statements for 3 (three) months;
- A copy of the Identity of Insured (KTP / SIM / Passport);
- A copy of the Identity of Heirs;
- Power of Attorney Form fill with medical record of the contents and signed on the stamp by Beneficiary / Insured;
- A copy of the HSBC Credit Card;
- The original certificate from the doctor who diagnosed for the first time;
- Examination results that support the diagnosis.
- Death claim, total permanent disability claim and total temporary disability claim must be made in writing to PT. Asuransi Allianz Life Indonesia within 60 days from the date of death or the date of declaration of total permanent disability or total temporary disability by the treating doctor.
- Critical Illness (CI) claim must be submitted to PT. Asuransi Allianz Life Indonesia no later than 30 calendar days after the diagnosis of the relevant critical illness.
- If a claim is to be found incomplete in documentation as required by item 1 and 2 above, PT. Asuransi Allianz Life Indonesia reserves the right to deem the claim to have never been made.
- PT. Asuransi Allianz Life Indonesia reserves the right to obtain any information / medical records from a Hospital and / or any other party in relation to the diagnosis and / or health services provided to the insured.
- If a claim is approved by PT. Asuransi Allianz Life Indonesia, the payment of insurance benefits in accordance with the Allianz aggreement will be made within 14 working days of receiving a complete document claim application.
